Name Change Applications
Whether you are recently married, separated/divorced, or seeking a change of name for personal reasons, our office can assist. We will review your circumstances, advise on the appropriate legal pathway, and tailor the process to your needs. Our services include preparing and completing all required forms, assisting you in locating and obtaining the necessary identification and certificates, and filing the application with the appropriate government authorities together with all supporting documentation. We manage the process end-to-end so it is handled accurately and efficiently.

Includes:
$137 Ontario government filing fee (Change of Name)
Commissioner of Oaths stamp for all required sworn declarations
Preparation of all forms, filing to the Office of the Registrar General, and status follow-up
Not included:
Police records check (if required by the Registrar General)
Provide your own or we can obtain it for $45 + HST (service fee; third-party police service fees, if any, are extra).

Adults (16+) who have lived in Ontario for at least 12 months may apply to legally change their name.
Youth 16–17 can apply, but written consent from every person with lawful custody is required; additional notices may be required.
Children (under 18): an application by or for a child generally requires custody-holder consent and, for children 12+, the child’s written consent (a court can dispense with consent in some cases, based on the child’s best interests).

If your change relates to marriage or separation, you may be able to assume or resume a last name without a legal name change. We’ll help you choose the simpler path where appropriate.

Proof of Ontario residency (12+ months) and a qualified guarantor;
Identity/civil status documents (birth certificate or other proof of legal name; marriage/divorce, if applicable);
Consents and notices required by law for youth/children;
Police record check if requested by the Registrar General.
